Cold abscess is a local manifestation of a condition which must be regarded as a general disease that is tuberculosis.

The development of cold abscess will usually be a chronic process with or without intercurrent or/and recent infection

Therefore the general treatment would be AKT, nutrition and protection from concurrent infections

The local mass lesion should be managed as per its situation and clinical scenario to prevent morbidity and to obtain best cosmetic result.

Chemotherapy includes rifampicin. INH, and pyrazinamide in first 2 months of intensive therapy. This will be followed by four months therapy with INH and rifampicin.Addition of etambutol or streptomycin will depend upon additional risk factors present .

As the mycobacterial population is relatively small in children isolated cold abscess in general are treated for 6 months and therapy beyond that is usually not required
